What is Form DS-11?

Form DS-11 is the Application for a U.S. Passport, issued by the U.S. Department of State. It's used by people who must apply in person — most often first-time applicants and children. You fill it out ahead of time, then submit it at an acceptance facility.

Who must use the DS-11?

You must apply in person with Form DS-11 if any of these apply to you:

  • You're applying for your first U.S. passport.
  • You're under age 16 (a parent or guardian applies with you).
  • Your most recent passport was issued more than 15 years ago.
  • Your previous passport was lost, stolen or damaged.
  • Your name changed and you can't document the change.

DS-11 vs DS-82 — which do I use?

Use the DS-11 when you must apply in person (first passport, child, or a passport that's too old or was lost). Use the DS-82 when you're eligible to renew by mail — generally if your most recent passport is undamaged, was issued in the last 15 years, and you were 16 or older when it was issued.

  • Your full name, date and place of birth, and Social Security number.
  • Your mailing address and contact details.
  • A physical description (height, hair and eye color).
  • Evidence of U.S. citizenship and a valid photo ID to bring with you.
  • One recent passport photo (2x2 inches).
